Parent/Guardian Waiver, Release of Liability and Indemnification Agreement
I the under signed parent/guardian of the above named minor who is part of the FTB Pros Baseball Program in conjunction with Linked Junction, LLC elect to allow my child and or children to participate in the FTB Baseball program at Eckerd College.
Furthermore, I acknowledge and am aware that there are certain risks and hazards involved in the above named program that may result in injury to the my child or children or other athletes including, but not limited to those hazards associated with this sport, playing conditions, equipment, and other participants.
I hereby release, discharge and agree not to sue FTB Pros, Linked Junction, LLC, or the organization's owners ,officers, agents, servants, associations, teams, affiliates, employees, volunteers or any person or entity connected with FTB Pros for any claim, damages, costs or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by the above named minor from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released.
I also hereby grant FTB Pros permission to use my child’s likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.
I understand and agree that all photos will become the property of FTB Pros and will not be returned. I hereby irrevocably authorize FTB Pros to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.
